East End CHC has served Toronto Beaches since 1990, focusing on the social determinants of health by providing holistic health care services to our community. East End CHC’s vision is Health and well-being for everyone in our community.

East End Community Health Centre

East End Community Health Centre (EECHC) is a Canadian nonprofit organization that provides primary care, health promotion, and disease prevention services to the south-eastern quadrant of central Toronto, Ontario. Established as a community health centre, EECHC focuses on delivering comprehensive health care to populations who experience barriers to accessing services due to factors such as language, culture, poverty, discrimination, or lack of health insurance.

Mandate and Services

EECHC is committed to reaching out to individuals in the community who may face difficulties in obtaining health care. Its approach emphasizes holistic care, recognizing the influence of social determinants of health—including income, education, relationships, and housing—on both physical and mental well-being. The centre’s health care professionals, including doctors and nurse practitioners, provide care that addresses these interconnected factors.

Services offered by the centre include:

  • Primary health care delivered by an interprofessional team
  • Health promotion programs tailored to community needs
  • Disease prevention initiatives
  • Support for populations with complex mental health issues and socio-economic challenges
Target Population

The centre primarily serves residents of the south-eastern quadrant of central Toronto and prioritizes populations who face systemic barriers to health care access. These include individuals living on low incomes, new immigrants, uninsured persons, and those affected by social isolation or cultural and language differences.

Organizational Structure

As a nonprofit, community-governed organization, East End Community Health Centre operates under the guidance of a volunteer Board of Directors. It is one of the 75 Community Health Centres in Ontario, which collectively strive to improve health equity by addressing social determinants of health and providing accessible, culturally sensitive care.

Hours of Operation

Typical hours of operation for the centre include extended weekday and limited weekend hours to accommodate community needs. After-hours access to medical consultation by phone is available for urgent health concerns.

Significance

East End Community Health Centre plays a vital role in Toronto’s health care landscape by supporting marginalized communities and enhancing access to primary care services. Its integrated approach to health care delivery exemplifies the community health centre model prevalent across Ontario, aimed at addressing health inequities and promoting well-being at both individual and community levels.

Business Registration

The centre is officially registered as a nonprofit organization under the Business Registration Number 129135604RR0001.
